02 | Submit the required documents based on the method of application:
o A U.S. government-issued social security number or a formal document from the U.S. Social Security Administration office indicating that you have applied for a social security number If a formal document from Social Security Administration is provided in lieu of a social security number, you have 30 calendar days to provide the social security number. If you fail to provide the social security number, the account will be closed. o A government-issued photo identification, such as passport, that is in compliance with the U.S. Patriot Act o Completed membership application form (available at the branch) with G4 staff member¡¯s name as sponsor to show membership eligibility o Certification of USD account (available at the branch) o $5.00 to deposit into the new account
o Application form with G4 staff member¡¯s name as sponsor, to be completed and submitted online. o Copy of a government-issued photo identification, such as passport, and $5.00 to deposit into the new account to be mailed to: |
Note: If the G4 staff member has a BFSFCU account, s/he can cash checks, wire funds to other bank accounts, apply for loans, and avail benefits that BFSFCU offers.
